START: Your first month will include weekly visits for medication injections, lab support, coaching, and encouragement.
MAINTAIN: After your first month, you’ll transition to at home treatments with monthly nurse practitioner check ins, guidance, and support.
RESULTS: Sticking to the customized care plan, and receiving medication as directed will lead to possibilities you’ve always wanted! Cancel anytime if not satisfied.

Frequently Asked Questions
How Do i Participate in Slenderize?
An initial consultation with our nurse practitioner is scheduled for an analysis of your body composition, lifestyle habits and goals. The results allow us to determine the right dosage of a breakthrough FDA-approved GLP-1medication called Semaglutide and the development of your customized weight management program
How Do Semaglutides Work?
GLP-1s help regulate and improve your body’s weight loss factors by regulating digestion and appetite, helping you feel full longer, which helps you avoid over-eating.
How Long is the Program?
Everyone is unique and weight management plans will vary. Slenderize has a required minimum 5-month commitment, but keep in mind that most people using GLP-1s have been shown to need about one year to lose 15% of their body weight. The first month in the Slenderize program includes weekly in-person medication dosing at our clinic. After your first month, you can transition to at home treatments but will still need to attend monthly nurse practitioner check ins for guidance and support. Once you hit your ideal weight, you can continue with maintenance doses or end your participation in the program. You can rejoin Slenderize by ilumaforma at any time if unwanted weight starts coming back.
How Fast Will I See Results?
As with any weight loss program, results vary, but most patients start experiencing weight loss within the first week. With proper diet and compliance with customized plans, patients have seen 2-3 lbs of weight loss per week.
How Much Does It Cost?
The one-time kick off fee for our supervised weight loss program, including initial labs, customized plan, in-office check-ins, medication & supplies, training, and nurse practitioner support is $699. Required weekly injections are $175 if done in-office or $150 if self-administered at home. Insurance assistance is not currently available.
